Mechanism

Metoprolol selectively blocks beta‑1 adrenergic receptors in the heart, reducing sympathetic stimulation. This decreases heart rate, myocardial contractility, and oxygen demand, improving cardiac efficiency and lowering blood pressure.

Pharmacokinetics

  • Absorption: Well absorbed orally; extended release ensures sustained plasma levels.
  • Distribution: Widely distributed; ~12% protein bound.
  • Metabolism: Extensively metabolized in the liver via CYP2D6.
  • Elimination: Excreted in urine as metabolites.
  • Half-life: ~3–7 hours, prolonged with ER formulation.

Side Effects

Interaction

  • Other antihypertensives (additive effect)
  • Digoxin (increased risk of bradycardia)
  • CYP2D6 inhibitors (increase plasma levels)

Warning and Precautions

  • Do not discontinue abruptly (risk of rebound hypertension/angina)
  • Monitor heart rate and blood pressure regularly
  • Caution in asthma/COPD patients
